Doula Services Agreement
Comprehensive legal terms: 3 prenatal sessions, 37-week on-call availability, continuous labor support, back-up guarantee, and $2,400 fee structure with 3 flexible installments.
HIPAA & Medical Privacy Release
Authorizes coordinated, respectful communication between Vanya and your OB/GYN, Certified Nurse Midwife, Labor & Delivery hospital staff, IBCLC lactation team, and pediatrician.
Medical History & Prenatal Intake
